 Orlando Hoop School – Youth Camp Waiver & Release Welcome to Orlando Hoop School Camps! We're excited to have your child join us for a fun, active, and safe experience. Please read the information below carefully before completing your registration.

 

Waiver of Liability By registering for the Orlando Hoop School Camp, I acknowledge and agree to the following: • I am the parent or legal guardian of the registered child.

 

• I understand that participation in basketball and physical activity carries a risk of injury, including but not limited to sprains, fractures, concussions, or serious medical emergencies.

 

• I release and hold harmless Orlando Hoop School, its staff, volunteers, coaches, and affiliates from any liability, claims, or demands related to injuries or property loss during participation.

 

 Medical Authorization I authorize Orlando Hoop School staff to obtain emergency medical treatment for my child if necessary. I agree to cover any related medical expenses and have disclosed all allergies or medical conditions in the registration form.

 

 Media Release I give permission for Orlando Hoop School to photograph and/or record my child during camp for use in promotional materials, including social media, websites, and advertising. (Photos may be used to highlight our camp community!)

 

 

 Code of Conduct I understand that campers must follow all camp rules, show respect for others, and maintain good behavior. Disruptive or unsafe conduct may result in immediate removal from camp without refund.

 

 Illness & COVID-19 Disclaimer I understand the risk of exposure to COVID-19 and other communicable illnesses. I agree not to send my child if they are experiencing symptoms or have been recently exposed. I acknowledge that Orlando Hoop School follows safety protocols but cannot guarantee a germ-free environment.

 

 Refund Policy I understand that all camp registrations are final unless otherwise stated. Credits or partial refunds may be issued for medical or emergency-related cancellations at the discretion of the camp director.
